AMDT CLASS POLICY

PLEASE READ, SIGN, DATE AND ADD THE CLASS YOU ARE ATTENDING

  • Participants must not have had any alcoholic beverages or be under the influence of drugs prior to attending class. Appropriate action will be taken. Visitors are not permitted in the class unless their attendance is approved by the instructor. A Participant has not completed the program until all class fees are paid in full. If the fees are not paid within 2 days of the class completion, a certificate will not be given and the Participant will need to repeat the class. There is a $25 fee for a lost certificate or lost receipt . Participants must attend all sessions in the order in which they are presented. If you miss a session, you will be required to pay a reschedule fee and you will be required to start the next available program over with the first class. If a second class is missed, the participant must pay in full for the next class. Reschedule fee: DWI $70, DOEP S75, Marijuana S75, Marijuana,Opioids, Alcohol and Other Drugs S85. There are no Refunds. Participants must complete all assignments and participate in class discussion appropriately to successfully complete the course. Failure to complete these will result in being dropped from the course and loss of all fees. Any disruption of the class will result in expulsion from the class and no refund. A minimum score of 70% on the written exam must be made to receive a certificate of completion. If you find you are unable to attend the classes on these dates, call at least 1 week in advance and you will be rescheduled, without cost, for the next session. If you register within the week of the class start date and don't show for class or cancel within the week of class, there are no refunds. Regardless of the reason, if you cancel less than one-week notice or do not show for the first day of class, you will need to sign up again and pay for the class in full. Refunds WILL NOT be given without 1 week prior notice. Credit Card transaction refunds will be assessed $15. Be on time! Doors will be locked and you will not be admitted if you are late. If you are late, you will be considered a "no show" and forfeit your seat and payment. No smoking or use of any tobacco products are permitted in the classroom or building. Smoking is permitted outside away from the doors to the building. A break will be provided to allow participants to smoke, use cell phones or use the restroom. Food and beverage is allowed during the class, however, please dispose of all trash properly. Cell phones are not permitted and must be turned off during class. NO TEXTING! Please respect each other, the Instructor and the facility; by not damaging equipment, defacing tables or chairs and refrain from abusive or inappropriate language. The Instructor or Administrator of the program can remove you. Weapons or anything that might be construed as a weapon is not allowed in the classroom or on the property where the course is conducted. NO EXCEPTIONS! I have read and understand the above policies and agree to comply with all items as listed. I further understand that any failure to comply with these policies will result in being dropped from the course and no refund will be given. No Refunds will be given for Evaluations/Assessments without a 24 hour notice of cancellation. A $25 reschedule fee will be assessed.

REMEMBER!  ARRIVE 30 MINUTES BEFORE THE CLASS START TIME TO FILL OUT A  PACKET THE INSTRUCTOR WILL HAVE FOR YOU.

There will be NO REFUNDS for Evaluations/Assessments without 24 hr notice. There will be a $25 reschedule fee without notice.
